I forgot my passcode

It can't be recovered, by you or by us. Uninstalling the app is the only way back in, and that deletes everything in it.

Your record stays on the old phone. Kithmark doesn't back itself up, so it can't move to a new one.

Open Settings in the app and choose Delete everything. It's immediate, and nothing can bring it back.
